What is a Screening???
Finding a great fit is vital in your Mental Health Journey. A screening is designed to identify your needs, acquire insurance information and discuss financial obligations. During this process our admin staff will ask a series of questions which are used to determine the next steps. Screening typically take 15 minutes to complete.
Below you have an opportunity to self schedule a screening or you can complete the "contact us" form and we will get back to you within approximately 48 hours.
What happens after the Screening???
After the screening you will receive two emails, one from Soul Discovery Admin Team and the other from SimplePractice. Both these email will provide you information on the next steps. Looking forward to working with you.
